It is important to let us know of any change of name, address or telephone number in order to enable your records to be kept up to date.

Due to pressures on General Practice our practice boundary was reduced in June 2010. Unfortunately, any patient changing their address beyond a radius of six miles will be advised to register with a GP in their new area.

If you have any queries, a member of our reception team will be willing to help you.
